Different sources of CSF leakage

Cerebrospinal fluid (CSF) leakage may occur in nasal, otological or spinal contexts after trauma or surgical procedures. Its source and clinical characteristics determine assessment and repair planning.

Dura and fibrin

Biological support in dural repair

The fibrin network of platelet gel provides a rationale for considering adjunctive use in dural repair. Clinical applications involving incidental dural tears have been reported. Platelet gel should not be assumed equivalent to approved fibrin sealants or suitable for every CSF leak.

Skull-base and intradural procedures

Skull-base surgery, acoustic neuroma surgery and intradural procedures are contexts discussed for platelet-based support. Use depends on the repair technique, the product’s applicable scope and the patient’s clinical condition.

Tumour surgery

Case-specific assessment rather than routine use

Preparations containing growth factors require separate assessment after tumour surgery. The Plateltex material does not recommend routine postoperative use to prevent CSF leakage after tumour surgery. The surgical and oncology team should consider product instructions and case-specific factors.
